Abstract
Maternal mortality is a major public health problem in developing countries. In the DRC, a woman dies every two minutes from obstetric causes. The rural health zone of Kikongo, in Kwilu province, is also affected. This study, conducted between 2020 and 2024, aimed to identify the determinants of maternal mortality. Data were collected through a desk-based survey using a questionnaire. Out of 28,069 deliveries, 68 maternal deaths were recorded, representing a frequency of 24.2 per 1,000. The main direct causes are hemorrhage (73.5%), obstructed labor (8.8%), postpartum infections (7.4%), and abortions (5.9%). Obstetric and gynecological determinants include prolonged labor, vaginal delivery, narrowed pelvis, precious pregnancy, scarred uterus, fetal distress, malpresentation, macrosomia, premature rupture of membranes, uterine rupture, circular cord, NIPT, placenta previa, myoma previa, and polyhydramnios. These factors highlight the need for enhanced prenatal monitoring and better obstetric care to reduce maternal mortality in this region.
